Release Form

In order to conduct a background check, the employer must have a completed Release Authorization and Application Form completed by the subject of the background check in their files. Click on the link below to download the form.

 

You may use your own release form if you prefer. Contact ADS for details on using your own form.

 

We must also have received a completed Client Agreement Form. Click the link below to download the form.

 

You will need the free Adobe Acrobat Reader software to view and print the ADS forms. Most computers these days already have the reader installed. If yours doesn't, a link to download the free reader software is included below.
